ICHOM 2025 Conference Abstract and Case Study Award Winners
At the ICHOM 2025 Conference in Dublin, we were proud to recognize a group of outstanding researchers, clinicians, and healthcare teams whose work is advancing value-based healthcare through measurable impact, collaboration, and real-world implementation.
This year’s Abstract and Case Study Award winners reflect the breadth and ambition of the ICHOM community. From predictive analytics at population scale to multidisciplinary care models and national implementations of outcome measurement, these projects demonstrate how data, clinical insight, and patient-centred design can translate into better care and stronger health systems.

Neural-Network Prediction of Lower-Extremity Amputation in an 837,402-Patient Population to Drive Value-Based Limb-Salvage Care
Voted for by conference attendees, this abstract stood out for its scale and ambition. By applying neural-network modelling across a population of more than 800,000 patients, the team demonstrated how advanced analytics can support earlier intervention, reduce avoidable amputations, and improve outcomes in limb-salvage care. It is a powerful example of how data science can directly inform value-based clinical decision-making.

Implementation of the ICHOM Obesity Standard Set in a Hybrid Weight Loss Program: 6-Month Outcomes from a Real-World Cohort

Early Impacts of a Community Pharmacist-Led Multidisciplinary Model of Obesity Care in Alberta, Canada

This project demonstrated the value of expanding obesity care beyond traditional clinical environments. Case Study Award – 3rd Place
Scaling Patient-Centered Outcome Measurement in Record Time: A National Implementation of ICHOM Sets in Brazil

This case study highlighted what is possible when outcome measurement is approached at national scale. The rapid implementation of ICHOM Standard Sets across Brazil demonstrated how alignment, leadership, and infrastructure can accelerate the adoption of patient-centred outcomes, enabling system-wide learning and improvement.

Transforming Total Knee Replacement Care Through a Systematic Value-Based Care Methodology: The One NHG Experience

Together, these award-winning abstracts and case studies demonstrate the growing maturity of value-based healthcare in practice. They show what is possible when outcome measurement is paired with clinical leadership, multidisciplinary collaboration, and a commitment to learning from real-world data.
