We’re excited to announce one of ICHOM’s new initiatives, as we lay the groundwork for a Cardiovascular Disease Learning Collaborative, which aims to standardize the measure of outcomes related to diseases that affect the cardiometabolic health of patients. We have chosen initially to focus on Coronary Artery Disease and Diabetes, as these are two of the most prevalent diseases globally, as well as two of our most implemented Sets across all regions.
We are identifying any interest by healthcare organizations or providers who are currently implementing one or both of the aforementioned Sets to potentially join this project. Our Learning Collaboratives (LC) aim to create a network of implementers committed to the practice of value-based healthcare in their institutions and whose primary focus is patient-centered care.
Forming part of our LC will connect you with like-minded healthcare professionals in different regions of the world and will allow you to improve your practice, through sharing of information, findings and learnings based on the implementation of our Sets, which have recently been revised and updated by a group of cardiology experts and patients from all over the world.
