ICHOM – the International Consortium for Health Outcomes Measurement

We would be grateful for your input, which we will use to develop a set of health outcomes for people with diabetes – The Diabetes Standard Set. The Standard Set will be used by healthcare providers around the world to provide clinicians and patients with information on the actual end-results of the care they are receiving. For more information about ICHOM, please visit our website, www.ichom.org.

Outcome Validation Survey aims to assess your perspective on the outcomes included in the ICHOM Diabetes Standard Set so far. The Standard Set has been proposed by the ICHOM diabetes Working Group, which is comprised of health professionals, and people affected by type 1 and type 2 diabetes. We are surveying people across the world to see if this list reflects the views of what matters most to people with diabetes.

The survey should take 5-10 minutes to complete. Your participation is on a voluntary basis.Your responses will be collected anonymously, and no identifying information will be collected.
